Creating a compassionate care coordination model for justice-involved individuals requires: 🔍 Identification of what a person needs upon leaving jail or prison; 🧰 Tools to connect a person with those needs; 🌐 Community resources that serve those needs. It was a great morning spent with the NC Department of Adult Correction team and the countless partners supporting the Reentry 2030 Initiative and the Governor’s Joint Reentry Council as a result of Executive Order 303. From the NC Department of Health and Human Services to the North Carolina Department of Commerce, it was wonderful to hear how so many agencies have come to the table to support those reentering society from North Carolina Facilities. Unite Us is a proud partner of this work as we jointly work to ensure every North Carolinian has access to valuable SDOH resources Special thanks to the Raleigh Police Department for opening up their training facility for the meeting as well as Todd Ishee and George Pettigrew for for driving this work forward!

📣 Introducing: a game-changer for Medicaid member eligibility verification and social care reimbursement. Today, we are thrilled to announce a new functionality enabling real-time Medicaid eligibility verification without leaving the Unite Us ecosystem! This new integration eliminates the need to access multiple systems and reduces the administrative burden of manually verifying Medicaid eligibility and enrollment each time services are needed. ❓Why does this matter? More than 77 million Americans rely on Medicaid for health insurance, but enrolling individuals in Medicaid programs, connecting them to care, and facilitating reimbursements for social care services can be challenging for organizations responsible for delivering these services. With this functionality in place, The Healthy Opportunities Pilot (HOP) launched by the NC Department of Health and Human Services has seen efficiency improvements in streamlining social care reimbursement workflows. 💬 “By leveraging this functionality, we can now allocate HOP’s resources more efficiently and effectively, ensuring that Medicaid members receive the support they urgently need in critical areas such as housing, food security, transportation, interpersonal safety, and toxic stress mitigation. This builds on the existing long-time partnership between HOP and Unite Us, underscoring our commitment to delivering targeted assistance where it matters most and ultimately improving health outcomes and fostering stronger, more resilient communities,” said Maria Ramirez Perez, Associate Director for Healthy Opportunities. Exciting News! 🎉 Today, we announced a new impact evaluation demonstrating the success of our Family Navigation partnership with Sarasota Memorial Health Care System and First 1,000 Days Suncoast. A few of the key preliminary findings include: 💡 An 89% improvement in parents' knowledge and awareness of concrete support, as well as a 35% increase in perceived access to social support during times of need 💡 A 54% reduction in perceived stress levels after 60 days 💡 An average increase in mentally healthy days by 4 days per month 💡 79% of referred cases were accepted by receiving organizations, with 63% ultimately closed as resolved “We're proud to partner with First 1,000 Days Suncoast to streamline referrals and foster collaboration among service providers,” said Dr. Amanda Terry, PhD, MPH, MA, Senior Director of Research and Evaluation at Unite Us. “Through our innovative, community-led partnership, we're transforming social care delivery and creating pathways to a brighter, healthier future for families during the critical early stages of life.
